on top of small 3' SMB and spool, whistle and backup light, I also carry in my "emergency pocket":

Trauma shears
Strobe
6' SMB
Dye
Mirror
Whistle #2

If heavy current will also attach DiverAlert LP horn, otherwise it's in pocket.
 
I carry al the above with 1 variation . A self inflate smb. I think this piece of equipment should be standard gear. In light of the 2 deaths recently in fla from a ooa situation this is self contained pull it crack it and off it goes . In a panic situation an oral inflate creates more task loading. With no air no way to inflate. Get back alive !
 
I've always called this collection of safety equipment a drift kit.
I carry this only when diving off a boat or in possibly unknown conditions.

No need in a small lake or qyarry.

My signal/lift bag and jump reel wit 100' of line are however always with me.

2. Working Light with fresh batteries Yes, during a night dive or wreck dive. Actually now that I think of it I always have one on me.
3. Reflective Signaling Device (mirror)Yes
4. Noise Signaling Device. Yes, a whistle.

Noise signalling device - Always (whistle and air siren on LPI hose)
