I dove on Mike Ball's Spoilsport about a year and a half ago and we were all required to have safety sausages when we dove. We were using their rental gear, and whistles were also attached to all their BCs. Dive Alert devices were not used, but I have one now on my personal gear and won't dive without it. I've heard that some liveaboards require them and this just seems like a good idea.

The Mike Ball trip was my first dive trip ever, and I must say I was very impressed with the safety measure taken. Like another poster stated, there was always someone on the top deck with binoculars watching for divers surfacing away from the boat and there were always divemasters in the water.
 
Diving in conditions beyond their ability? :idk:

Lacking a decent surface signaling device? :idk:

Plain old bad luck?

without knowing more then read here sounds like a bit of all three...
