It fits a regular printer sized page pretty well. Just download the jpeg and print, and if you like it you get a free log book for life, well almost free.

That question is way beyond a scuba board post but if you PM me with an Email Address I can create a virgin copy of what I have an email it to you.

The construction of an electronic dive log would be better suited in a database, such as Access or SQL, as opposed to a spreadsheet.
