I cannot afford Trimix for a 100' dive, and until I have any incling of danger on air above 200', I'll continue to dive it.

Trimix for a 200+ foot dive costs more than for a 100 foot dive (more Helium), so I doubt you'll be able to afford it even if you get that inkling.

As far as medical insurances go, what about obese people that drive up these costs? What about smokers/drinkers?

As far as I'm concerned, if you choose to not work, society shouldn't have to foot the bill your your living expenses. Same thing with smokers/drinkers. If you choose to poison yourself, society shouldn't have to foot the bill for your associated healthcare. Obesity is a tough one since it is at least partially genetic.

One of the most contributing sources of obesity is eating a lot of free carbohydrates (those that are not surrounded by fiber) which drives hi level of insulin which in turn drives fat depositing and killing pancreas leading to diabetes. Why are so many North Americans obese ? Look at what people eat. They consume high level of free carbohydrates like sugar in the coffee, junk white bread like wonder bread, they eat low fat yogurts that contain 20-30 % of sugar !!! French fries (potato has a lot of free carbohydrates ) Coke (30 gram of sugar I think per a can) and other junk.
Look at what is sold in the stores all kind of cookies have up to 50%!!!! percent of sugar. The sugar is everywhere in large quantities. Manufactures add more sugar as people's tastes adapt to it and they do not feel it anymore so they add more. After I was avoiding eating sugars for some time and I ate one donut I got such a heavy sugar rush that thought I would have to call the ambulance. I became so dizzy.

If you eat meat (a lot of calories) alone or with vegetables you will not get obese believe me - I lost 20lbs eating just meat and vegetables in 3 month. So the calories have nothing to do with obesity , it's another BS fed to us. The body dump all the calories that are not consumed.

And those on the lower end of the economic scale are least likely to be able to afford nutritious food. The cheapest and most plentiful foods are simple carbohydrates.
 
Same thing with smokers/drinkers. If you choose to poison yourself:
Good in theory. But it means that society (healthcare, whether public or private) should only foot the bill for those who have absolutely no vices at all. What % of the population would that be, and who would decide what % of doing/eating what would be harmfull enough to cut you off from healthcare?

I'd be willing to limit it to those things that there is clear evidence and consensus are damaging. Tobacco companies aside, most people agree that smoking causes lung cancer. If your vice is drinking iced tea, and there are some unknown long-term medical issues associated with drinking iced tea, that's different.

I'm not going to even attempt laying out a plan to implement something like that. Just talking general viewpoints here.
